Position SummaryThe LPN promotes the provision of resident care, promotes a team approach with the Resident Assistants, Certified Nursing Assistants, and other team members in Activities, Housekeeping and Dining Services. The LPN provides direct care as well as directs and oversees care provided by the caregivers to ensure the Resident’s Plan of Care is followed. An active approach to resident care will be taken and not just passive maintenance.
LPN’s will be deeply involved in resident activities and programming and must understand and have a desire to grow in dementia-specific care.
- Support the maintenance of wellness objectives, standards of wellness practice, and wellness policies and procedure manuals.
- Identify ongoing needs and services of residents through the assessment/Service Plan process including documentation for residents with a change in health care status.
- Respond promptly to resident needs.
- Assist Residents with activities of daily living as needed, including bathing/showing according to schedule, oral care, toileting, dressing, etc. Perform restorative nursing techniques including proper positioning, ADL training, and passive range of motion, and provide behavioral prompts and affirming support.
- Maintain a restraint‑free environment at all times and support fall prevention programs for all residents in need.
- Assist in post‑mortem care as instructed.
- Communicate appropriate and timely information to families; report concerns to supervisor/Director of Nursing and share with team members to improve care delivery.
- Oversee and manage medication delivery system including timely order entry, receipt, tracking, daily MAR documentation, narcotic count, and checking medications to ensure resident compliance.
- Maintain infection control policy and procedures, update manuals, and keep supplies on hand.
- Willingness to work beyond normal hours and assume other positions temporarily when necessary.
- Coordinate with the Food Service Director to ensure residents receive prescribed diets.
- Maintain documentation reflecting wellness care provided and resident responses.
- Arrange for qualified team members to assume duties during Director of Nursing’s absence.
- Participate in interviewing and selecting residents for admission to meet placement needs.
- Maintain open, effective communication with the community’s medical staff.
- Ensure a safe and sanitary environment for residents, team members, and visitors.
- Keep Director of Nursing informed of performance or incidents.
- Ensure compliance with safety and infection control policies, encourage handwashing and proper use of PPE.
- Maintain confidentiality of all resident and organizational information.
- Demonstrate good judgment, strong problem‑solving, and decision‑making skills.
- Attend in‑service and continuing education required for professional certification or licensure; maintain professional affiliations.
- Perform other duties as assigned.

Educational Requirements- High School diploma or equivalent (GED).
- Current Licensed Practical Nurse License in the state of Tennessee.
- Demonstrate competence in assessment skills, injections/medication administration, and triage.
- Maintain current unrestricted state license as a Licensed Practical Nurse or a Professional Registered Nurse per state regulations.
