Licensed Practical Nurse – LPN (Full‑Time)

The LPN provides quality patient care and performs skilled nursing services in patients’ homes. Responsibilities include following state laws and the plan of care, coordinating with the healthcare team, and ensuring a safe home environment.

• Deliver skilled nursing care within the scope of practice as defined by Tennessee state laws.
• Assist and educate patients’ families in patient care and home maintenance.
• Observe, document, and report patients’ responses to treatment and any changes in condition daily to the supervising nurse or physician.
• Collaborate closely with physicians and registered nurses, documenting care accurately and promptly.
• Maintain the integrity of the plan of care and refrain from altering it without authorization.

• Minimum one year of clinical LPN experience.
• Current LPN license in the state of service.
• Current CPR certification.
• Valid driver’s license, vehicle insurance, and reliable transportation or access to public transportation.
• Ability to commute to multiple patient residences daily within the assigned geographic area.
