Overview
The Weekend Treatment Nurse is responsible for providing high-quality wound care, skin assessments, and treatment services to residents while ensuring compliance with physician orders, regulatory requirements, and facility policies. This position plays a key role in promoting wound healing, preventing skin breakdown, and maintaining accurate documentation throughout weekend coverage.
Responsibilities- Perform all ordered wound care and treatment procedures according to physician orders and established clinical standards.
- Complete comprehensive skin assessments on admission, readmission, return from hospital, and as assigned.
- Monitor wound progression and promptly report changes in resident condition to the physician and nursing leadership.
- Collaborate with the Wound Care Nurse, DON, ADON, MDS Coordinator, therapy, and interdisciplinary team to develop and implement individualized care plans.
- Ensure all treatments are documented accurately and timely in the electronic medical record.
- Maintain treatment carts and supplies, ensuring cleanliness, organization, and appropriate inventory levels.
- Photograph wounds per facility policy and maintain accurate wound measurements and staging.
- Assist with pressure injury prevention initiatives, including resident positioning, support surfaces, and skin protection interventions.
- Educate nursing staff, residents, and families regarding wound care, skin integrity, and prevention strategies as appropriate.
- Monitor for signs and symptoms of infection and communicate concerns promptly.
- Participate in infection prevention efforts related to wound care.
- Ensure compliance with CMS, state, and facility regulations regarding treatment documentation and skin management.
- Provide direct nursing care and assist with resident needs as necessary to support quality resident outcomes.
- Participate in weekend clinical rounds and communicate resident concerns during shift report.
- Perform other duties as assigned by nursing leadership.
- Current unrestricted RN or LVN/LPN license in the State of Texas.
- Current CPR certification.
- Long-term care experience preferred.
- Wound care experience preferred; wound care certification is a plus.
- Knowledge of skin integrity, pressure injury prevention, infection control, and documentation requirements.
- Ability to prioritize workload and work independently.
- Excellent communication and organizational skills.
- Ability to stand, walk, bend, stoop, and lift up to 50 pounds with assistance as needed.
- Ability to push treatment carts and transport supplies throughout the facility.
- Visual acuity sufficient to perform detailed wound assessments and documentation.
- Saturday and Sunday
- 8-hour or 12-hour shifts (based on facility needs)
- Occasional holiday coverage may be required.
