Deliver educational workshops, life skills training, youth development sessions, and family engagement activities.
Monitor participant attendance, participation, and progress toward program goals.
Ensure curriculum implementation aligns with program requirements and established quality standards.
Adapt instructional techniques to meet the diverse needs of participants.
Provide ongoing case management services to assigned youth and families.
Conduct participant assessments to identify strengths, needs, barriers, and service requirements.
Develop and monitor individualized service plans designed to support participant success.
Maintain regular contact with participants and families to monitor progress and provide support.
Advocate on behalf of youth and families to ensure access to appropriate services and resources.
Facilitate internal and external referrals to community resources and support services.
Establish and maintain collaborative relationships with schools, social service agencies, healthcare providers, and community organizations.
Follow up on referrals to ensure participants receive needed services and support.
Assist families in navigating community resources and accessing available benefits and programs.
Perform all other duties as outlined by supervisor.
