** Home health visits in
** Hendry county** (including
** Clewiston, LaBelle, Lehigh Acres, Moore Haven, Buckingham**, and surrounding areas)
** Schedule**:
Full-Time; M-F 40 hours with required weekend/holiday rotations as needed
** What you'll Do
*** Provide physical therapy services to patient as prescribed by a physician in compliance with agency policies and procedures and federal and state regulations.
* Perform initial and ongoing assessments, including OASIS assessments at appropriate time points, to determine level of functioning and patient’s ability to perform activities of daily living.
* Develop and revise plans of care, in consultation with the physician and other care team members, which will enable patient to achieve realistic goals of mobility.
* Reports to the physician and designated agency representative patient’s reaction to treatment and any changes in the patient’s condition.

** What You'll Need
*** Active Florida PT license
* Minimum of 1 year experience as a licensed Physical Therapist (PT).
* Home health care experience strongly preferred.
* Strong assessment, critical thinking, and patient education skills
