RN Case Manager; Cypress, FL

Our client is seeking an RN Case Manager located in Clewiston, FL or Fort Pierce, FL to evaluate patient care in both inpatient and outpatient settings. This role is responsible for developing care and discharge plans, performing utilization and medical necessity reviews, advocating for patients and families, and ensuring high-quality, cost-effective care delivery. This is a full-time, on-site position with a Monday-Friday, 8:00 AM-5:00 PM schedule.

• Provides high-quality, patient-centered care by developing, coordinating, and evaluating individualized care and discharge plans.
• Manages complex outpatient needs.
• Conducts prospective, concurrent, and retrospective clinical reviews to ensure medical necessity, appropriate utilization of services, and cost-effective care.
• Advocates for patients and their families across inpatient and outpatient settings.

• 2 years of case management experience.
• ASN required.
• Florida RN license required.
• BLS required.
• FL Driver's license required.
• Certification in Case Management, or ability to obtain within 1 year of employment required.
