Job Description:

Schedule:

5 days per week, 40 hours per week, Monday to Friday, 7:00 am to 3:00 pm, plus lunch break, no weekends Setting:
Multi-specialty surgery center OR Facilities: 7 OR???s, 2 procedure rooms Supervision: MD???s on-site supervise CRNA???s Cases:
General, Ortho, Plastics, ENT, GI, GYN, Eyes, Pediatrics Provider Ratio: 2-3 docs on-site; CRNA's 8-10 daily Charting:
Paper charting, no EMR Daily Cases:
Range anywhere from 45-80 per day Duration: 3 months Requirements:
Board Certification: NBCRNA, ACLS required License Requirement:
Must have Active FL license Privileges: 60 days, Clean Files Only Anesthesia Modality Checklist:
Completed by the provider within 6 months CV:
Must be in complete month/year format covering all gaps of 30 days or more NPDB Self-Query:
Required for all providers who have malpractice claims or background issues Completed EVPS Disclosure Form Clinical Interview:
May be required Certification Requirements: ACLS NBCRNA State License Requirements:
