What You Need to KnowWe are looking for a qualified Nurse Practitioner to join our hospice team in Edison, NJ. If you meet these qualifications, we would love to meet you:
- Graduated from an approved school of professional nursing – Master’s degree
- Current, unencumbered advanced practice nurse license in the state(s) practicing
- One year of experience as an NP
- Certified by a nationally recognized advanced nurse practice organization
- Valid CPR certification while employed in a clinical role (preferred)
- Licensed driver capable of traveling to all business locations
- Meet state regulations and requirements for the program’s services
- Initiate communication with attending physicians, hospice staff, and other agencies to coordinate optimal care for the patient and family.
- Monitor patient progress; update the Team Director and members; consult regarding discharge planning and patient/family teaching.
- Maintain continuity of care with the care team and case manager for patients with ongoing needs.
- Prescribe medication as permitted by state and/or the state Board of Nursing.
- Conduct face‑to‑face clinical and needs assessment at intake, preferably at the patient’s home, covering medical, emotional, spiritual, social, and practical needs.
- Reassess as needed and at least every 60 days; develop plans of care based on assessments and the patient’s and family’s priorities.
- Initiate communication with attending physicians, hospice staff, and other agencies to coordinate optimal care.
- Supervise the care plan, including all medication orders.
- Collaborate with the pre‑hospice team, alerting them to changes and newly identified patient needs as they arise.
