Lead Outpatient Palliative Care Nurse Practitioner / Physician Assistant

A growing, mission-driven palliative care organization is seeking a Lead Advanced Practice Provider (NP/PA) to oversee and mentor a team of APPs delivering outpatient palliative care in New Jersey.

• Lead, mentor, and support a team of Nurse Practitioners and Physician Assistants
• Provide outpatient palliative care consultations and follow-up visits in skilled nursing facilities and community settings
• Guide goals-of-care discussions and advance care planning with patients and families
• Promote clinical quality, documentation excellence, and evidence-based best practices
• Assist with onboarding, case reviews, quality initiatives, and clinical escalation
• Develop relationships with skilled nursing facilities and community partners throughout the region

• Active New Jersey Nurse Practitioner or Physician Assistant license
• DEA and CDS (or ability to obtain)
• 1+ years of clinical experience in palliative care preferred, or hospice, geriatrics, oncology, internal medicine, family medicine, hospital medicine, critical care, skilled nursing, or complex chronic care
• Previous leadership, mentoring, precepting, or supervisory experience preferred
• Excellent communication skills and a passion for improving patient care
