Registered Nurse Case Manager - Field in Essex & Counties

The Registered Nurse Case Manager position is a field/remote role focused on delivering quality healthcare to members in Essex County and surrounding areas. Ideal for experienced nurses, this role involves significant travel and requires strong clinical judgment and communication skills.

• Pay: $66,575 - $142,576/year
• Location: Remote, US
• Residency: Must reside in Essex & surrounding counties in New Jersey
• Schedule: Monday - Friday, 8am-5pm EST
• Travel: Up to 75% travel in Essex and surrounding counties

• Conduct face-to-face visits in members' homes using comprehensive assessment tools.
• Evaluate medical needs of members enrolled in Managed Long-Term Services and Supports (MLTSS) and Dual Special Needs Program (D-SNP/FIDE).
• Facilitate members' overall wellness by addressing requests for various services.
• Schedule and attend interdisciplinary meetings advocating for members' safe discharge.
• Develop care plans and authorize services within MLTSS/FIDE benefits.
• Document accurately and timely in the member’s electronic health record.
• Coordinate care with the member, authorized representative, and care team participants.
• Mentor new hires once proficiency is attained in the role.

Education:

• Minimum associate’s degree or diploma in Nursing required

Experience:

• 3+ years clinical practice experience in a hospital or alternative care setting

Licenses &

Certifications:

• Active and Unrestricted RN license in NJ required
• Successful completion of the NJ Choice certification is a contingency of employment
