SUMMARY:
The Intensive Case Manager (ICM) is a compassionate, organized, and self‑directed individual who works under the supervision of the Regional Program Coordinator and provides case management services for people referred through the Camden County Board of Social Services. The ICM attempts to engage, assess, and intervene with clients who are nearing the end of their lifetime benefits of Temporary Assistance to Needy Families (TANF) and/or General Assistance (GA) with a goal of assisting them to obtain employment or to identify whether a client is unable to be employed for reasons related to health, educational, social, or other significant barriers.
The ICM meets with clients in‑person a minimum of two times per month for as many as 24 months to assist the client with their self‑identified goals. The ICM works in collaboration with the Work First New Jersey case managers at the Board of Social Services to ensure the clients’ needs are being addressed holistically. The ICM is also responsible for timely and clearly written professional documentation that communicates the clients’ needs and the interventions employed to address those needs, to the State Division of Family Development (DFD) that funds the SAIF program.
ESSENTIAL DUTIES AND RESPONSIBILITIES Reflect the mission and vision of Catholic Charities in every encounter with and on behalf of clientsReceive and review new client intake information from the County Social Services Agency on a monthly basisEnsure that clients are aware of their scheduled intake, and subsequent appointments, through the use of phone, electronic, or postal communicationConduct in‑person comprehensive client social assessment for each client referred to the ICM each monthAssist clients in the development of a strengths‑based individualized service plan (ISP) that seeks to move the client toward employment or to address employment barriersEngage in motivational interviewing interventions that inspire clients to comply with SAIF program requirementsAssess clients’ barriers toward employment and make necessary referrals to assist in addressing those barriersConduct outreach to clients a minimum of twice monthly to ensure movement toward the goals they have set in their ISP; make home visits as necessary to engage clientsComplete all associated documentation required by DFD and by Catholic Charities in required time frames utilizing the forms provided by DFD and using professional documentation standardsDocument all in‑person and phone contacts with client such that it adequately and accurately reflects the interaction, progress toward the clients’ goals, challenges in accomplishing the goals, and next stepsShare needed resources and referrals to clients to help them address employment barriersCommunicate to WFNJ case managers any client non‑participation in program activities using appropriate communication meansComplete a monthly report that will be submitted to DFD that documents all efforts to communicate with clients and the progress toward the clients’ goalsMonitor clients’ time in the program and ensure that clients are aware of their remaining time and the consequences of reaching time limitAttend meetings with State and County officials related to client time limits and be a fair and strong advocate for clients during those meetingsAssist in the planning and actualization of activities for clients that assist in moving them toward self‑sufficiencySeek out and share employment opportunities for clients; provide or identify employment support to clients on an as‑needed basisAssist and advocate for clients with healthEnhance professional growth and development through participation in educational programs, emergent literature and research, in‑service seminars and workshops, etc., in accord with professional licensure standards and requirements.The Intensive Case Manager (ICM) for Gloucester County must have reliable transportation in order to travel and meet with clients, ensuring that clients do not experience hardship due to lack of transportation or financial limitations.EDUCATION AND/OR EXPERIENCE
