Advisa Care is seeking a Full Time Hospice Case Manager to join our compassionate team serving Flint and surrounding areas. The RN will assess patient needs, develop care plans, and support families as they navigate end-of-life care with an interdisciplinary team.
Ideal candidates have an active Michigan RN license, hospice experience, strong communication, and a willingness to travel for home visits. Weekend on-call duties are required on a rotating basis and a reliable vehicle is necessary.
