ResponsibilitiesDuring a visit of up to one hour, providers review and observe a member’s current health, medical history, medication adherence, social environment, and other risks. The Matrix Comprehensive Health Assessment (CHA) involves:
- Conducting adult/geriatric assessments, including medical history, diagnosis, treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines.
- Administering and ordering point-of-care or lab-based testing as needed.
- Working with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.
- Collaborating with the Primary Care Physician (PCP) on patient education and follow-up.
- Providing services at the member’s place of residence, which may be their home or a skilled nursing facility.
- Flexibility - Visits are scheduled based on your availability. We conduct assessments Sunday through Saturday from 8 am to 7 pm, allowing you to work during the day, evening, or weekend.
- Competitive Pay - Earn income based on the number of assessments you complete. Increase your earning potential by offering greater availability and traveling to see members outside your home location. Those completing a monthly average of 20-100 visits may earn between $1,800 and $11,500 per month, based on an assessment completion rate of $90-$115 depending on your state of residence.

- Additional Licensure - Support for obtaining additional state licensure and credentialing in neighboring states when needed.
- Full-time Opportunities - PRN providers can transition to full-time roles, including a full-time sign-on bonus.
- Master’s Degree or commensurate experience and satisfactory completion of NP or PA licensure.
- Current NP or PA licensure in state of practice, including prescription authority or the ability to obtain prescriptive authority.
- Board certification by the NCCPA, AANP, ANCC, or AACCN in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty.
- Current BLS, ACLS, or CPR certification.
- 1 year experience as a Nurse Practitioner or Physician Assistant preferred; new graduates encouraged.
- Valid state driver’s license and proof of adequate automobile insurance coverage for the state of residence.
- Strong computer skills and familiarity with employee health/medical record software.
- Excellent verbal and written communication skills.
