About This Role
As a Home Health Registered Nurse, you will provide and direct nursing care to patients in their homes, coordinating with healthcare professionals to ensure continuity of care. This role is ideal for nurses who are passionate about making meaningful connections with patients and their families.
Highlights- Pay: $67,905 - $101,857 annual total cash target pay, $32.65 - $48.97 hourly rate, $39.18 - $58.76 per visit point

Full Time
- Initiate, develop, implement, and revise the plan of care in collaboration with the physician and other healthcare professionals.
- Supervise care provided by home health aides and licensed practical/vocational nurses, providing instruction and assigning tasks.
- Provide required supervisory visits.
- Complete comprehensive assessments (OASIS) including medication reconciliation accurately and timely.
- Document patient visits per policy and payer requirements, syncing timely per LHC policy.
- Ensure patients meet home health eligibility and medical necessity guidelines.
- Communicate relevant information timely and effectively with appropriate agency staff.
- Assist in the orientation of new agency personnel and serve as a preceptor to other staff and students.
- Participate in on-call and weekend rotation as needed to meet patient needs.
- Adhere to and participate in the agency's utilization management model.
- Current and unrestricted RN licensure in State of Mississippi
- 1+ years of Home Health experience preferred
- Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
- Current CPR Certification or ability to complete within 90 days of hire preferred
