Family Centered Treatment Specialist

Health Connect America is seeking a Family Centered Treatment Specialist to provide intensive in home and community based behavioral health services to children and families. In this role, you will conduct assessments, develop and implement family centered treatment plans, deliver counseling and skill building interventions, and coordinate care with schools, agencies, and community partners. You will document services, monitor progress, and participate in multidisciplinary team meetings.

Responsibilities

• Provide in-home and community-based family-centered treatment services to children and families.
• Conduct biopsychosocial assessments and develop individualized treatment plans.
• Deliver evidence-based counseling, skill-building, and crisis intervention services.
• Coordinate care and communicate with schools, agencies, and community partners.
• Maintain accurate, timely clinical documentation in EHR systems.
• Monitor client progress, adjust interventions, and participate in case reviews.
• Collaborate with a multidisciplinary team to support client goals and safety.
• Educate families on coping strategies, parenting skills, and community resources.
• Participate in ongoing training and professional development.
• Ensure compliance with agency, payer, and regulatory standards and policies.
