Overview

At Liberty Hospice we understand the unique needs of our patients and families facing terminal illness. That is why Liberty Hospice provides our hospice patients with state-of-the-art care and pain management services, delivered by our specially trained staff with emphasis on strength, dignity and compassion.

• Implements palliative care program requirements
• Completes medical visits with patients per guidelines and coordinates care with the Palliative Care team as clinically indicated
• Conducts face-to-face encounters to provide recertification information to Medical Director/Associate Medical Director for hospice patients
• Provides clinical findings of the face-to-face encounter to the primary care physician and/or supervising Medical Director/Associate Medical Director
• Communicates with patients/families at all levels
• Communicates with all agency personnel

• Master’s degree in Nursing required, with current licensure in the state of practice
• Certification as an Adult or Family Nurse Practitioner
• Experience in Hospice or Palliative Care nursing preferred
• Knowledge of the framework, function, and organization of a hospice care program
• Excellent communication and organization skills
