Job Description:

Medical Hematology/Oncology coverage for 4/5 days per week. Inpatient coverage of both sites

Number of Physicians: 1

Number of Advances Practice Providers: 0

Supervision of APPs expected: NONursing Support: 1:1

Medical Imaging:
Yes Lab amp Pharmacy:
Yes Clinic Provider Daily Volume: 10-30

Average Daily Hospital Practice Census: 10-15

Appointment Length: consult - 40 minutes follow up - 20 minutes

Patient Population:
Primary Clinic - Medicare/ Medicaid/Commercial Secondary Clinical - Commercial Medicare

EMR: EPICServices offered in the practice
Patient assessments new treatment plans ongoing treatment plans changes in therapy referral for ancillary services end of life care chemotherapy management pain management radiation and surgical oncology management provided by existing providers with strong relationships

Bill Rates Needed for Submission:
Malpractice Hourly Holiday  Hourly Night Call to include 2 gratis hours

Holiday Night Call
24 Hour On Call to include 4 gratis hours

Holiday 24 Hour On Call Call Back Holiday Call Back Conversion fee 25k rate cap Travel Reimbursement:
Flights - 750/round trip cap to include all taxes and fees. No upgraded airfares or seat upgrades

Lodging - 1.5x the GSA rate cap average for the total stay to include taxes and fees Rental Car - 55/day cap including all taxes and fees regardless of car class. SUV/upgrades over cap will need to be preapproved will only be approved if needed for inclement weather

Mileage - IRS Standard Rate
