Responsibilities

Provide courteous, high-quality customer service by greeting clients and visitors, assessing needs, and directing them to appropriate staff, programs, services, or community resources.

Serve as an initial point of contact for clients by conducting phone and in-person intakes, gathering information, assisting with forms, and facilitating access to services.

Explain available Tribal, state, federal, health, and human services programs, including mental health, medical, dental, substance use, social services, income maintenance, public health, and other community resources.

Coordinate client intake, enrollment, and assignment processes to ensure timely access to services and a smooth transition to case management and other support programs.

Meet with clients to collect, verify, and submit required documentation and determine initial and ongoing eligibility for programs and services.

Collaborate with case managers, supportive services staff, and other team members to identify client needs, coordinate services, and address immediate concerns.

Identify individuals who may be experiencing a crisis or urgent need and connect them with appropriate interventions, services, and resources.

Maintain complete, accurate, and confidential client records, files, documentation, and electronic data entry in accordance with program requirements, best practices, and privacy standards.

Complete intake forms, program documentation, eligibility records, activity records, and other required reports to meet Tribal, state, and federal requirements.

Provide administrative and office support, including answering phone calls, processing incoming and outgoing mail, faxing and distributing documents, scanning, photocopying, filing, typing, word processing, and electronic documentation.

Compose, edit, and prepare correspondence, newsletters, articles, brochures, forms, reports, and other program-related materials.

Create, maintain, and update databases, tracking systems, and data entry processes to support program operations and reporting requirements.

Organize and maintain client files, office records, and work spaces to ensure an efficient and orderly work environment.

Assist Adult Case Managers, Supportive Services staff, and other department personnel with direct client services, projects, transportation, and program activities.

Follow program policies, procedures, and guidelines to ensure compliance with applicable Tribal, state, and federal regulations.

Maintain confidentiality of client information and records at all times

Appropriate work attire is required.

Must attend all mandatory department trainings.

Ensures confidentiality of financial records and all records for employees and/or clients.

Displays a responsive and professional manner in promptly responding to all requests, complaints, and problems.

Recognizes that each employee is a representative of the Fond du Lac Band of Lake Superior Chippewa and is responsible for demonstrating courtesy, respect, and sensitivity to the needs of all others, including visitors and co-workers.

Represents the Fond du Lac Band of Lake Superior Chippewa and the department in a positive and professional manner in the community.

Required to maintain proper attendance including reporting to work on time in accordance with applicable policies.

Maintains a clean and organized work area.

Due to changes and modifications in the job from time to time, employees are required to be flexible and assume other responsibilities assigned by management as deemed necessary.

Qualifications

Valid Driver's License issued in the US is required.

Eligibility determined by FDL's insurance company.

High school diploma or GED required.

A.A degree is preferred.

Knowledge of computer applications to include word processing, data bases and/or data entry, Spreadsheets, and Microsoft Outlook is required.

Knowledge of Native American communities, culture, customs, and beliefs is preferred.

Experience working with Native Americans clients, communities and is able to demonstrate cultural sensitivity is preferred.
