Position: Psych Nurse Practitioner New Mexico

Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering residential, outpatient, inpatient, patient‑centered medical homes, medication‑assisted treatment, and crisis services throughout Arizona. The agency serves individuals experiencing crisis, opioid use disorder, homelessness, and mental illness.

Position Overview

Psychiatric Mental Health Nurse Practitioner (PMHNP) – part‑time, onsite; ages 15 and up, primarily adults. The schedule is Monday, Wednesday, and Friday 8:00 AM–5:00 PM.

Shifts
  • PMHNP/FNP – Crisis Sobriety Center (AM/PM shifts Sunday–Tuesday; rotating on Wednesdays or Thursdays‑Saturday)
  • PMHNP – SMI Caseload, full time on‑site, Mesa location; 8:00 AM–4:00 PM Monday‑Friday
  • PMHNP – ACT/FACT, full time on‑site, Central Phoenix; 8:00 AM–5:00 PM Monday‑Friday
Core Responsibilities
  • Provide patient‑centered, evidence‑based psychiatric nursing care to adults 18+ in the inpatient setting.
  • Perform urgent psychiatric and/or addiction medicine evaluations for patients referred to the 23‑hour observation unit from hospitals, urgent care, law enforcement, and other medical practitioners.
  • Complete urgent psychiatric assessments, ambulatory detox, or physical health assessments for patients identified by CBI screening staff.
  • Coordinate admission, transfer, and discharge with other CBI programs to ensure seamless transitions and accurate medical record documentation.
  • Initiate or maintain doctor‑to‑doctor communication and document all care coordination appropriately.
  • Conduct daily patient evaluations and adjust treatment and discharge planning as needed.
  • Implement seclusion and restraint interventions in line with CBI policy, including assessment, documentation, and timely order entry.
  • Prepare discharge orders; review cases and discharge plans during rounds with assigned medical staff; sign orders within 24 hours of discharge.
  • Complete all required application documents for involuntary evaluation and emergency admission within two hours of receipt and notify admission coordinators or medical director as appropriate.
  • Maintain a minimum 85% compliance with documentation requirements, a 75% follow‑up rate, and a 85% patient satisfaction score; submit correction plans if thresholds are not met.
  • Adhere to all CBI policies, procedures, and scheduling protocols; maintain a minimum 60‑day notice for time off and coordinate coverage with peers.
  • Participate in quality management initiatives, state or accreditation surveys, peer review activities, utilization reviews, and performance improvement projects.
Universal Requirements & Continuing Competence
  • Obtain orientation, including shadowing another CBI practitioner, before patient contact.
  • Complete a TB skin test prior to working with patients and annually thereafter; submit results to the Credentialing department.
  • Acquire and maintain a suboxone waiver to treat the maximum number of eligible patients.
  • Document and renew CPR/First Aid certification within the required two‑year period; submit proof to Credentialing.
  • Use e‑prescribing for all patient prescriptions as established in the CBI EMR.
  • Attend regular staff rounds, education sessions, and quarterly practitioner meetings; provide 24‑hour notice if unable to attend.
