MDS Coordinator

We are seeking a highly organized and detail-oriented professional to join our team and lead our clinical assessment and care planning efforts.

• Coordinate and facilitate the development of the Minimum Data Set (MDS) for each resident, ensuring accuracy, completeness, and compliance with regulations
• Collaborate with the interdisciplinary team to identify resident needs and develop individualized care plans
• Conduct thorough assessments and evaluations to identify resident goals and outcomes
• Maintain accurate and detailed records of resident assessments, care plans, and progress
• Develop and implement strategies to improve quality of care and resident outcomes
• Participate in facility-wide quality improvement initiatives and projects
• Maintain knowledge of relevant regulations, policies, and procedures
