Role Overview

The purpose of a Physician Assistant at Oak Street Health is to provide effective and equitable value‑based primary care to adults on Medicare, keeping them happy, healthy, and out of the hospital. Physician Assistants see patients independently and collaborate with physicians, depending on the patient’s specific situation. They provide longitudinal care and build meaningful relationships that foster trust and help patients understand and feel connected to their care plan.

They practice in outpatient clinics on collaborative care teams consisting of a physician, nurse practitioner or physician assistant, medical assistants, a dedicated medical scribe, and support from registered nurses and care managers. They partner with elite specialists and hospitals for specialty and acute care, enabling providers to focus on primary care, care coordination, and population health.

Core Responsibilities
  • Provision of exceptional primary care
  • Conduct office visits for routine and acute issues
  • Administer annual wellness visits and health risk assessments, requiring a holistic view of health and thoughtful, accurate documentation
  • Coordinate care with other providers, specialists, testing facilities, and agencies
  • Lead population health initiatives, e.g., ensuring all eligible females receive evidence‑based breast cancer screening every two years
  • Assist the care team with phone triage and outreach
  • Educate patients on health conditions, care plans, and treatments
  • Participate in Oak Street Health promotional activities
  • Conduct home visits as needed
  • Perform other duties as assigned
Required Qualifications
  • 3 years of primary care experience (preferred)
  • Active, non‑probationary state Physician Assistant license
  • Active DEA license
  • U.S. work authorization
  • Demonstrated commitment to Oaky values
Preferred Qualifications
  • Experience in primary care: internal medicine, geriatrics, or family medicine
  • Understanding of HCC documentation, ICD‑10 coding, and Health Risk Assessments
  • Passion for teamwork and cross‑functional collaboration
  • Desire to be part of an innovative, empirically‑guided population health model
  • Bilingual proficiency, where applicable
Pay Range

$87,035.00 – $ (typical pay range for this role)
