Title: Licensed Practical Nurse - LPN
Pay Rate: $40 - $45 per hour
Location: Las Cruces, New Mexico
Employment Type s: Full‑Time or Part‑Time available
Availability: Must have weekday and/or weekend availability.
Shifts: 8‑hour shifts; schedules are subject to change based on operational needs.

Position OverviewYou will be providing health assessments and home health visits for our patients.
Job Duties- Drive to patients’ homes and conduct home health visits independently
- Obtain and document medical histories, obtain consent, check patient history, and administer immunizations
- Complete documentation of assessment and/or interventions
- Respond to minor troubleshooting of test systems and take appropriate action according to predetermined protocols
- Identify problem that may adversely affect test performance or reporting of test results and notify supervisor
- Perform all relevant diagnostic tests as required
- Perform venipuncture as required
- Complete accepted medical forms
- Conduct weekly equipment checks in exam rooms and report any malfunctions
- Code medical exams as needed
- Administer vaccines and medication to patients
- Measure vital signs, monitor patient progress and changes in behavior
- Recognize and treat any medical condition with appropriate techniques, equipment, and knowledge of all state and county protocols
- Recognize situations requiring immediate physician or emergency care attention
- Interact with patients and families compassionately
- Interact with clinicians, other health professionals, and staff courteously and professionally
- Communicate effectively with patients and agencies
- Participate in ongoing professional development activities
- Maintain a positive and professional work environment
- Maintain the confidentiality of all personal health information as dictated by HIPAA laws
- Complete other duties as assigned
- LPN License
- Blood withdrawal / phlebotomy experience
- Minimum of 6 months of LPN experience
- BLS certification from the American Heart Association
- Valid driver’s license in state of residency with 2+ years of driving experience and an acceptable driving record
- Excellent verbal communication skills
- Ability to lift 50–60 lbs
- Patient bedside experience preferred (not required)
- Fluent bilingual English/Spanish preferred
- Comfortable driving and making independent home health visits
