Responsible for utilizing the nursing process to provide direct patient care to an assigned group of patients during a shift. Responsible for prioritizing the delivery of direct nursing care using time and resources efficiently integrating a standards-based framework model.

- Act as clinical resource by overseeing patient care delivery, directing and delegating assignments to home health aides and nursing staff based on acuteness and staff capability, anticipating future needs.
- Maintain competency in providing direct patient care, evaluate outcomes, consult with specialists, adjust nursing care processes as indicated to ensure optimal patient care.
- Coordinate patient care through collaboration with the patient, family, all other disciplines, primary and secondary physicians and community resources/programs.
- Demonstrate knowledge, philosophy and provide support to patients and their families.
- Ensure patients understand their rights, responsibilities, advanced directives and meaning of informed consent during admission.
- Assess and direct patient and family teaching needs, proficient in use of return demonstration and teaching aids in the home setting.
- Maintain professional responsibility for knowledge of current nursing research, practice standards, guidelines, and protocols; hospital policies, procedures, and programs to improve patient care quality.
- Maintain proficiency in using all equipment used in practice.
- Effectively communicate and assist other members of nursing team and/or other disciplines; participate in problem solving and conflict resolution to maintain team cohesiveness.
- Perform on‑call duties effectively, triage patients over phone and visit when needed.
- Direct own educational needs for top 10 ICD‑9 codes/diagnoses: diabetes, wounds/post‑surgical, arthritis, newborns, Coumadin dependency, malnutrition/nutrition needs, medication instruction, pain and symptom control/hospice care.
- Supervise home health aides in compliance with Medicare/Medicaid rules and regulations.
- Perform other functions as required.
- Degree from an accredited academic nursing program; if under conditional status, approval may be granted by system CNO.
- Minimum ADN preparation.
- State of New Mexico or Compact State Nursing License required.
- Required certification from AHA, ASHI, or Red Cross.
- BLS required for all RN roles.
- ACLS required in Emergency Department, Critical Care, Cardiovascular Lab, GI Lab, Interventional Radiology, Progressive Care, Surgical Services Pre‑Op and Recovery Room.
- NRP required in Family Birthing Center, Labor and Delivery, Neonatal Intensive Care, Mother/Baby Care.
- PALS required in Children Services, Cardiovascular Lab, Emergency Department, GI Lab, Interventional Radiology, Surgical Services Pre‑Op and Recovery Room.
