Licensed Vocational Nurse

Genesis HCC, part of Genesis Health Care, is seeking Licensed Vocational Nurses to provide compassionate, patient-centered care in our skilled nursing and rehabilitation center. As an LVN, you will deliver direct bedside care, administer medications and treatments, monitor resident conditions, and document accurately in the EHR. You will collaborate with an interdisciplinary team, supervise CNAs, support rehab and long-term care goals, and uphold infection control and regulatory standards.

Responsibilities

• Provide direct nursing care to residents in a skilled nursing and long-term care setting
• Administer medications and treatments as prescribed and document accurately
• Monitor, record, and report changes in residents' conditions to RNs and physicians
• Assist with admissions, transfers, and discharge planning
• Collaborate with interdisciplinary team to implement individualized care plans
• Support residents with daily living activities while promoting dignity and independence
• Educate patients and families on care plans, treatments, and health maintenance
• Maintain infection control, safety, and regulatory compliance standards
• Supervise and direct CNAs and other support staff as assigned
• Participate in quality improvement initiatives and ongoing clinical training
