LVN​/LPN – LTC​/SNF

Job Summary:

Assist in delivering high-quality patient care within a Long-Term Care/SNF setting as an LVN/LPN. Responsible for providing foundational nursing services and supporting the healthcare team to ensure optimal patient outcomes.

Responsibilities:

• Administer medication and treatments according to physician orders and facility protocols.
• Monitor and record patient vital signs, symptoms, and responses to treatments.
• Assist with patient assessments and report significant findings to the supervising nurse or healthcare team.
• Support patient daily activities, including hygiene, mobility, and comfort measures.
• Ensure compliance with infection control and safety procedures in accordance with healthcare standards.
• Document patient care accurately and thoroughly in medical records.

Qualifications:

• LVN or LPN license with valid state registration or licensure.
• Proven experience in a LTC/SNF environment preferred.
• Ability to prioritize tasks and work effectively in a fast-paced setting.
• Excellent communication and interpersonal skills.
• Knowledge of state and federal healthcare regulations concerning nursing care.
• Must bring a valid  up-to-date COVID-19 vaccination documentation.
