LVN​/LPN - LTC​/SNF

Job Summary:

Provide skilled nursing care to residents in a Long-Term Care or Skilled Nursing Facility setting. Deliver compassionate patient-centered services ensuring safety, comfort, and well-being of all patients.

Responsibilities:

• Administer prescribed medications and treatments accurately and timely.
• Monitor and document patients’ vital signs and health status regularly.
• Assist with activities of daily living, including bathing, dressing, and mobility support.
• Collaborate with healthcare team to develop and implement individualized care plans.
• Ensure compliance with facility policies, safety standards, and infection control protocols.

Qualifications:

• Hold a valid LVN or LPN license in the state of practice.
• Demonstrate at least 1 year of experience in LTC/SNF environments.
• Possess current CPR and BLS certifications.
• Exhibit strong communication, assessment, and interpersonal skills.
• Ability to work flexible shifts, including weekends.
