Job Summary

Support Services Navigator will play a key role in supporting individuals living with dementia. This position involves conducting phone and in‑home assessments with patients and families to determine eligibility for government‑funded programs. Once members are enrolled, the Support Service Navigator will maintain ongoing engagement by completing monthly phone check‑ins to ensure continued support and address any questions or needs.

Job Duties
  • Assist with reviewing and evaluating enrollment packets for participants and attendants.
  • Self‑schedule assessments and phone check‑ins.
  • Provide individuals with care plans and supporting resources as applicable.
  • Conduct in‑home safety assessments by visiting members’ homes as needed.
  • Promote cohesiveness in the office.
  • Collaborate with stakeholders regarding the delivery of services.
  • Comfortable with public speaking.
  • Comply with applicable legal requirements, standards, policies, and procedures.
  • Conduct advanced reporting and oversight activities.
  • Demonstrate effective problem‑solving and decision‑making skills.
  • Handle complex service programs and special projects.
  • Maintain necessary skills and knowledge to coordinate workflow.
  • Participate in professional development and training activities.
  • Prioritize and multitask effectively.
  • Provide excellent customer service to internal and external clients.
  • Represent the company at stakeholder meetings, health fairs, and provider fairs.
  • Oversee deliverables and ensure timely completion of projects.
  • Enroll clients and employees in services, web portal, and EVV systems as needed.
  • Provide hands‑on mentorship and coaching on in‑person office days to support staff development.
  • Conduct data collection and analysis to ensure accuracy for reporting purposes.
  • Bilingual preferred in various locations.
  • Other duties as assigned.
Qualifications
  • Bachelor’s degree in Social Work, Psychology, Counseling, Rehabilitation, Nursing, Sociology or Related Field preferred.
  • 3 years’ experience working with individuals with disabilities or the elderly preferred.
  • Combination of education and experience.
  • Ability to work flexible and/or extended hours, if needed, to meet the job requirements.
  • Employees must provide their own reliable internet if working a remote/hybrid position.
