About the Role:

We are seeking a skilled and compassionate LPN to join our care team at our facility in Clyde, OH. As an integral member of our nursing staff, you will provide direct patient care, administer medications, and monitor residents' conditions to ensure they receive the best possible care. Your attention to detail, strong communication skills, and ability to work effectively in a fast-paced environment will be essential in providing optimal care to our residents.

Responsibilities:

• Provide direct patient care, including bathing, dressing, and feeding residents

• Administer medications and treatments as prescribed by physicians

• Monitor and record vital signs, such as blood pressure, temperature, and pulse

• Develop and implement individualized care plans for residents

• Communicate effectively with residents, families, and other healthcare professionals

• Participate in care conferences and contribute to the development of new care plans

Requirements:

• Current LPN license in the state of Ohio

• High school diploma or equivalent required

• 1 years of experience preferred, but not required
