• Salary Range: RN- $38.00- $57.00 per hour, LPN- $31.40-$47.10 per hour

• Provides safe, direct physical care of the resident.
  • Maintains a clean, safe and orderly resident environment by keeping the area clean and disposing of soiled, contaminated items.
  • Reports malfunctioning, broken, or hazardous equipment or conditions to the supervisor.
  • Observes policies and procedures as demonstrated through performance of procedure and tasks.
  • Conducts and documents a thorough assessment of each resident's medical status upon admission and throughout the resident's course of treatment.
  • Assists in the development and implementation of an individualized treatment plan for each assigned resident.
  • Completes required documentation in an accurate and timely manner.
  • Oversees care delivery by Licensed Practical Nurses and Geriatric Nursing Assistants.
  • Attends and/or participates in rehab team conferences regarding resident progress, problems and needs.
  • Acts in compliance with Broadmead regulatory and professional standards and guidelines.
  • Assesses continuing education opportunities appropriate to discipline and responsibilities in order to maintain clinical currency.
  • Adheres to facility policies and procedures and participates in facility quality improvement and safety programs.
  • Performs other duties as assigned.
  • Accountable for promoting internal customer service.
