Job Summary
The Licensed Practical Nurse (LPN) provides direct patient care in a long-term care (LTC) setting within a healthcare facility. This role focuses on delivering high-quality nursing care, supporting residents' daily health needs, and ensuring compliance with clinical standards and individualized care plans.
Key Responsibilities
- Provide direct nursing care to residents in a long-term care environment.
- Administer medications and treatments as prescribed.
- Monitor patient conditions and report changes to the Registered Nurse or physician.
- Maintain accurate and timely documentation using Point Click Care EMR.
- Assist with activities of daily living (ADLs) while ensuring patient comfort and safety.
- Collaborate with interdisciplinary teams to implement and update care plans.
- Adhere to established policies, procedures, and regulatory requirements.
- Active Licensed Practical Nurse (LPN) license (in good standing).
- Minimum 2 years of experience in Long-Term Care (LTC) or a similar setting.
- Experience with Point Click Care (PCC) preferred.
- Ability to work 12-hour shifts (days or nights) and cover holidays.
- Strong patient care and clinical assessment skills
- Medication administration proficiency
- Documentation and EMR system experience
- Effective communication and teamwork
- Ability to manage time in a fast-paced environment
