What You ll Do:

• Conduct comprehensive patient assessments and develop individualized care plans.
• Coordinate with interdisciplinary teams to ensure seamless, high-quality care.

Educate patients and families on managing health at home.

Document care with accuracy, compassion, and compliance in mind.

Schedule:

Full-time, day shift

Compensation:
Competitive salary based on experience

Bonuses:
Relocation and sign-on bonuses available (case-by-case)

What You ll Bring:

Active RN license in Idaho (or compact state)

At least 1 year of home health experience

Strong communication and organizational skills
