Job Summary
Optum House Calls is searching for a House Calls Advanced Practice Clinician (Nurse Practitioner) for a hybrid, full‑time role with 25% statewide travel. Expect all travel expenses to be covered. A flexible schedule between 8am and 8pm across a 7‑day week is offered, with no on‑call, weekend, or holiday requirement.
Primary Responsibilities- Conduct comprehensive in‑home health assessments for Medicare Advantage and other plan members.
- Review past medical history, reconcile medications, take vital signs, and perform physical examinations.
- Execute evidence‑based screenings and point‑of‑care testing when appropriate.
- Identify and document diagnoses for care management and treatment planning.
- Communicate findings to members’ primary care providers to address gaps in care.
- Recognize urgent or emergent situations and intervene accordingly.
- Educate members on disease processes, medications, and treatment compliance.
- Address social determinants of health and provide referrals as needed.
- Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult‑Gerontology Primary Care.
- Active, unrestricted licensure in the state of practice (or ability to obtain by start date) with no disciplinary actions or practice limitations.
- Active, unrestricted driver’s license and reliable transportation for home visits.
- Ability to navigate varied home environments and to sit, stand, or kneel during assessments.
- For states allowing independent NP practice, obtain board approval if required within the specified timeframe.
- At least one year of clinical experience in family, geriatric, or home health care.
- Proficiency with electronic medical records and related technology.
- Ability to transport equipment up to 30pounds and access stairs during visits.
- Strong communication skills with geriatric or Medicare populations.
- Salaried range: $91,700 to $163,700 annually, adjusted for local market, education, experience, and certifications.
- Student loan repayment (up to $40,000) or $25,000 sign‑on bonus.
