LV HOME CARE AND HOSPICE HOME CARE CASE MANAGER (REGISTERED NURSE - RN) – Full Time, Day Shift
$10,000 SIGN ON BONUS.
SummaryResponsible for coordinating and directing the delivery of care for an assigned caseload of patients receiving services in the home setting in collaboration with the interdisciplinary care team. Determines appropriate level of care and skill needs based on Conditions of Participation, initiates, reviews, evaluates, and revises care plans with the physician and team to achieve patient goals, quality metrics, and level of care transition.
Performs assessments, provides skilled treatments, and delivers patient and caregiver education.
- Coordinate and direct care for assigned patients in the home setting with the interdisciplinary team.
- Initiate, review, evaluate, and revise established care plans in collaboration with the physician and team to progress toward patient goals.
- Determine appropriate level of care and skill needs following Conditions of Participation.
- Relay significant changes in patient status to the physician and team in a timely manner.
- Deliver patient care per physician's treatment plan using a patient‑family centered approach.
- Provide education to patients, families, and staff on end‑of‑life issues, palliative care, advance directives, chronic disease management, pain management, symptom control, home care, hospice, and discharge planning.
- Promote patient and caregiver autonomy and evaluate effectiveness of teaching, modifying education as needed.
- Maintain accurate CMS‑regulation data collection, reflecting quality outcomes and appropriate reimbursement.
- Specialized diploma from an NLN approved/accredited nursing program.
- One year recent direct‑care experience in a healthcare setting.
- Knowledge of patient education techniques and principles.
- Ability to take initiative and work independently without direct supervision.
- Excellent time‑management and organizational skills.
- Capacity to manage a caseload of patients per agency standards.
- Ability to travel to patient care assignments.
- Knowledge of Conditions of Participation and regulatory data‑set collection.
- High skill in verbal and non‑verbal communication.
- Ability to facilitate difficult and complex communication related to goals of care.
- Basic Life Support (BLS) certification.
- Registered Nurse licensure in PA upon hire.
- Driver's license (PA or NJ) upon hire.
- Associate’s or Bachelor’s degree in Nursing.
- One year experience as a home health or hospice RN.
- Experience navigating multiple computer applications and EMR platforms.
- Ambulatory Care Nursing‑Board Certified (AMB‑BC) within 3 years or Certified Hospice and Palliative Nurse (CHPN) within 3 years.
- Lifting and carrying 40‑50 lbs, including pushing/pulling patients on bed or stretcher, lateral transfers, standing, walking, and possible CPR.
- May need to kneel, bend, squat; ability to walk 8–10 miles per shift.
Day Shift – Mon–Fri, 8AM–4:30PM (weekends, holidays, on‑call rotation possible). 175EBrown
