Kootenai Health seeks an experienced RN Care Coordinator to partner with a multidisciplinary team in CdA to manage chronically ill patient populations. You will guide planning, education, and ongoing care across the continuum, aligning with patient goals and health outcomes.
You will lead care coordination efforts, educate patients and families, and ensure documentation and communication are maintained within the EHR. A BSN and RN licensure are required; multi-state licensure preferred.
