An exciting opportunity has arisen to join an innovative team within the Serious Mental Illness (SMI) and frailty pathway in NE Essex. We are looking to recruit a motivated, hardworking team leader who has excellent communication skills with a passion in driving service improvement and working collaboratively with other health care providers and third sector agencies. The successful candidate will be responsible for the delivery of a high level service in providing a proactive assessment, monitoring and management of patients over the age of 70 years with an SMI and the management of the team.
As part of the national agenda for transformation the service will focus upon admission avoidance to both general hospital and a mental health inpatient setting by integrated working with the community teams in line with the North East Essex Alliance.

The focus of this role is to lead, support and facilitate the development of integrated Neighbourhood SMI and SMI with Frailty pathways. Working in partnership with the wider NEE Alliance health and social care economy including third sector organisations.
