Requirements:
- AHA BLS certification required
- Minimum of 3 years of prior community hospice experience
- Excellent typing skills for narrative heavy EMR
- EPIC experience within the past 2 years required
Roles & Responsibilities :
- Provide end-of-life care assessment and care planning with a focus on comprehensive symptom management.
- Initiate and participate in goals of care discussions with patients, families, and the healthcare team.
- Administer medications via various routes (IV, Subcutaneous, oral, etc.) and manage IV access devices.
- Educate patients and caregivers on aspects of end-of-life care and medication management.
- Perform clinical skills such as wound care, pain management, and tracheostomy care.
Compliance / Notes:
- Orientation: 2-3 days
- Dress Code:
Business casual or non-hospital scrubs in non-institutional colors; closed-toe supportive shoes; limited jewelry; hair and nails must be neat and tidy.
