Registered Nurse - Case Manager - Community Hospice

Responsibilities

Join our Community Hospice team as a Case Manager Registered Nurse and build lasting connections with patients and their families. You will deliver skilled nursing care, educate patients and families, and create a supportive environment that honors their wishes.

This is an in‑person position involving case management and performing home visits within the service area of Chittenden and Grand Isle counties. On‑call:
Not typically, variable depending upon program needs.

• 2 years’ experience in either home health, home hospice, or med‑surg (or a combination of these three settings).

• Current RN licensure recognized by the State of Vermont.
• Appropriate experience in specific clinical area. Varies by unit.
