** Assistant Clinical Manager** (ACM) is a
** Registered Nurse
** leadership role that supports Home Health & Hospice Clinical Managers and Directors in delivering safe, compliant, high-quality home health and hospice care across community-based services.
The ACM provides clinical oversight during assigned coverage (including weekends/holidays and off-hours as needed), supervises and schedules designated staff, coordinates care delivery and handoffs, and serves as a point of escalation for clinical and operational concerns.
The role partners closely with interdisciplinary and administrative teams to ensure continuity of care seven days a week and adherence to organizational policies and applicable regulations.

** EDUCATION
* ** Associate or Bachelor’s degree in Nursing from an accredited program.
* Current, unrestricted RN license in the state of practice and BLS certification.
* Hospice and Palliative Nursing certification (preferred) or willingness to obtain within 1 year
** EXPERIENCE
* ** Three years’ experience as a Registered Nurse in Home Health and/or Hospice/Palliative care;
* Two years progressive leadership/supervisory experience. Consideration may be given for a combination of education and experience from which comparable knowledge and skills are acquired
* Demonstrate and apply working knowledge of hospice/home health regulations, standards of practice, and clinical workflows to promote compliance, efficiency, and high-quality outcomes;
Apply clinical judgement to balance patient needs, regulatory requirements and organizational policies in care planning and delivery.
* Maintain productivity and quality of work while managing frequent interruptions and multiple concurrent responsibilities
* Demonstrate effective written and verbal communication skills when interacting with staff, leadership, patients, and interdisciplinary partners.
