Essential Job Functions
  • Manage the health issues and provide nursing care to clients. Observe and monitor client health; document treatments and/or medical concerns in client charts.
  • Perform a variety of duties associated with maintaining optimal health for clients, including administration of medications, obtaining prescribed medications from a pharmacist, administering medications to clients, providing information about medications, obtaining medications for home administration, monitoring medication changes, and observing client reactions to new medications.
  • Attend to clients who admit themselves as being sick (Sick call) or injured.
  • Assist with psychological reviews; schedule client appointments with health care providers and dentists. Attend all neurology/specialty appointments as appropriate.
  • Contact parents or legal guardians to discuss client medical issues, status of health conditions, and treatments received.
  • Work on‑call duty in rotation as appropriate; promptly respond to calls for immediate nursing services.
  • Provide hepatitis B or flu vaccines and PPD testing to Grafton staff.
  • Conduct in‑service staff training related to nursing and health issues; teach medication administration and related medication issues and practices to staff.
  • Promote and assist in the control of communicable disease.
  • Provide the medical perspective into client care for all multidisciplinary team (MDT) meetings.
  • Participate in various facility‑wide meetings and committees on an as‑needed basis.
Additional Job Functions
  • Serve as backup for nurses in other Grafton locations as needed.
  • Prepare and/or complete a variety of health services forms such as medication permission forms, on‑call sheets, medical/dentist appointment sheets, etc.
  • Perform other work as assigned.
Minimum Training and Experience
  • Graduation from an accredited nursing program.
  • 1–2 years of experience providing nursing services; or any equivalent combination of training and experience which provides the required knowledge, skills, and abilities.
  • Must have a valid state license as an RN.
Schedule
  • Every Wednesday overnights 7:00 pm–7:00 am.
  • Every other weekend Saturday and Sunday 7:00 am–7:00 pm.
  • Other shifts if needed.
Special Requirements

Licensure as a Registered Nurse; and a valid driver’s license with an acceptable driving record.

Compensation
  • Starting at $38.46 up to $51.39; commensurate with experience and qualifications.
Employee Benefits (PRN / Part‑time)
  • 401(k), including an employer match up to $2,500 annually.
  • Education Assistance up to $5,250 annually (must work an average of 20 hours a week).
  • Employee Assistance Program (EAP).
  • Referral Bonus Program – Employees receive $1,000 for referring new employees to Grafton ($500 paid upon hire and $500 paid after 90 days of employment).
