Summary of Position: The Chaplain is responsible for providing spiritual counseling and emotional support to the active and bereaved patients and families. They will serve as a liaison between community clergy and Ever Heart Hospice.
Education and Experience:
- Bachelor level degree from an approved Theological Seminary or School of Divinity preferred.
- Clinical Pastoral Education (CPE) accreditation preferred.
Special Skills and Requirements:
- Shall have a valid driver’s license.
- Detail oriented with excellent communication, documentation, and organizational skills.
- Ability to work independently without supervision.
- Travel to other sites as needed. Travel may be in inclement weather.
Responsibilities of Position:
- Provide a comprehensive spiritual assessment and create individualized goals and interventions for patients and families in accordance with the plan of care.
- Provide spiritual care to patients and families who request spiritual support visit(s). Spiritual care may include counsel, prayer, Bible reading and/or sacraments or other rituals as appropriate .
- Give respective spiritual support that is consistent with patient and family cultures, religions, and values.
- Participate in IDG meetings and contribute information concerning the spiritual needs of patients and families with the interdisciplinary group.
- Provide education to employees, volunteers, and the community regarding appropriate spiritual care and support and its role in hospice and palliative care.
- Work evenings and weekends as needed to meet the needs of the patients, families, groups, and community. ( ie . funerals, grief support groups, community presentations)
- Assist with grief support groups for bereavement services as directed by the Director of Patient and Family Services.
- Complete all documentation as required by the organization in compliance with Agency policy, accreditation bodies, and governing requirements
