Position Summary
This is a full-time telework role for a Certified Diabetes Care Education Specialist Registered Nurse. Candidates must reside in a compact state and hold a compact license. The working schedule includes Monday‑Friday standard business hours with two evening shifts per week (11:30 am‑8 pm CST) and rotating Friday and Saturday shifts approximately twice per quarter following training.
Responsibilities- The Health Coach Consultant utilizes a collaborative process of assessment, planning, implementation, and evaluation to engage, educate, and influence members’ decisions related to achieving and maintaining optimal health status for chronic conditions.
- Assesses members using clinical tools and information/data review, conducting comprehensive evaluations of members’ needs and benefit plan eligibility for available integrated internal and external programs/services.
- Utilizes assessment techniques to determine members’ level of health literacy, technology capabilities, and/or readiness to change.
- Applies and/or interprets applicable criteria and guidelines, health/wellness management plans, policies, procedures, and regulatory standards to enable appropriate utilization of services and integration with available internal/external programs.
- Consults with supervisors, Medical Directors, and others to overcome barriers to meeting goals and objectives.
- Identifies and escalates quality of care issues through established channels.
- Utilizes influencing/motivational interviewing skills to ensure maximum member engagement and promote lifestyle/behavior changes.
- Provides coaching, information, and support to empower members to make ongoing independent medical and/or healthy lifestyle choices.
- Interprets and utilizes clinical guidelines/criteria to positively impact members’ health, providing up‑to‑date healthcare information to facilitate understanding of their health status.
- Helps members actively and knowledgeably participate in healthcare decision‑making with their provider.
- Develops and monitors established plans of care, in collaboration with the member and/or attending physician, to meet members’ goals.
- Utilizes internal policy and procedure in compliance with regulatory and accreditation guidelines.
- Must hold a CDCES (Certified Diabetes Care Education Specialist).
- Active and unrestricted RN licensure in a compact state, holding a compact license.
- 3+ years of clinical experience.
- Must possess or be willing and able to obtain high‑speed broadband internet access.
- Previous coaching experience.
- Managed care experience.
- Experience with Microsoft Word, Outlook, Excel, and comfort using various computer programs.
- Willing to obtain additional nursing licensure.
- Associate’s degree required; BSN preferred.
