Patient Services Coordinator

What You’ll Do
  • Coordinate and schedule patient visits using our electronic medical record system
  • Partner with nurses, therapists, and the interdisciplinary team to ensure seamless patient care
  • Monitor reports and proactively follow up on incomplete or untimely visits
  • Collaborate with field clinicians and leadership to optimize productivity, coverage, and scheduling efficiency
  • Utilize analytics to reduce missed visits and support balanced caseload distribution (RN/LVN, Therapist/Assistant, FT/PRN)
  • Communicate clearly and professionally with patients, staff, referral partners, and leadership
  • Support quality initiatives, including participation in QAPI programs
  • Promote Vital Caring’s philosophy and standards of care in every interaction
What You’ll Bring
  • At least one year of general office experience (healthcare or hospice preferred)
  • High school diploma or equivalent (18+ years of age)
  • Strong organizational skills and attention to detail
  • Ability to multitask in a dynamic clinical environment
  • If clinical or making patient visits: current state license (if applicable), valid driver’s license, auto insurance, and CPR certification
