Adaptive Home Health is seeking a Licensed Vocational Nurse (LVN) Field Clinician to deliver skilled nursing visits in patients’ homes across Texas. You will work under RN/clinical supervision, perform focused assessments, and communicate changes promptly to the care team.
You will document thoroughly in Homecare Homebase, support medication reconciliation, wound care, and patient education while traveling across the assigned territory.
Schedule:
full-time with some weekend coverage.
