Skilled Nursing; LPN or RN Collegeville

We are seeking Licensed Practical Nurse or Registered Nurse to work one-on-one with our pediatric client in an in-home setting in the Collegeville, PA area.

Shift availability:
Monday through Friday, or Saturday and Sunday. Additional clients in surrounding area. Full‑time or part‑time positions available.

• Must be a caring and reliable individual who can work in a diverse atmosphere
• Assess patient’s need for safety measures and appropriate interventions
• Implementation of the Nursing Care Plan as outlined by the Primary Nurse
• Implement medication changes according to verbal and/or written physician orders
• Administer prescribed medications and treatments
• Accurately observe and promptly report changes in the patient’s physical or mental status to the nursing supervisor
• Accurately record nursing observations, activities, and treatments on the appropriate forms
• Ability to follow a physician's plan of care under the supervision of our Director of Nursing
• Communicate with family members, office staff, and all supporting health professionals

• Current Pennsylvania skilled nursing license
• Current CPR certification
• Able to lift at least 50 pounds
• Pass state and federal criminal background checks
• Up‑to‑date physical and two‑step PPD or chest X‑ray
• Pass skilled nursing pre‑employment test
• Form of transportation to client’s residence
