Responsibilities & Leadership Duties

• Flex Coverage:
  Rapidly deploy to cover open shifts or initiate care with new clients across our service area to guarantee a flawless start of care.
• Mentorship & Training:
  Provide on-site coaching, support, and relationship-building visits to our care staff.
• Care Plan Management:
  Observe clients for changes in condition, communicate updates to the Client Care Coordinator, and brief caregivers before they start a new case.
• Clinical Documentation:
  Maintain accurate care notes, ADLs, and employee feedback logs within Axis Care in strict compliance with HIPAA laws.

Requirements

• Active Pennsylvania CNA License is preferred
• 4+ years of professional caregiving experience (Home Health Aide or equivalent).
• Previous healthcare management or supervisory experience is highly preferred.
• Valid driver's license, reliable auto insurance, and willingness to drive clients within our service area.
• Complete availability for a 7-day on-call rotation at least once per month.
• Willingness to provide or obtain a CPR Certification and a TB Test Result
