The Medical Records / Case Manager is responsible for overseeing medical record accuracy and compliance while also coordinating resident care, services, and discharge planning. This role supports interdisciplinary communication, survey readiness, and continuity of care by ensuring documentation meets all federal, state, and facility standards. The position works closely with nursing leadership, therapy, social services, and external providers to promote quality outcomes for residents.
Job Responsibilities- Maintain complete, accurate, and compliant medical records in accordance with federal, state, and facility regulations.
- Ensure confidentiality of resident information and adherence to HIPAA standards.
- Monitor documentation for accuracy, timeliness, and completeness to support survey readiness.
- Coordinate resident care plans in collaboration with the interdisciplinary team.
- Facilitate admissions, transfers, and discharge planning to ensure smooth transitions of care.
- Communicate with residents, families, insurance providers, and external agencies as needed.
- Track and manage required documentation related to case management, care coordination, and resident outcomes.
- Identify opportunities for process improvement to enhance documentation accuracy and care coordination.
- Support quality assurance and performance improvement initiatives.
- Minimum of one year of related experience preferred.
- Previous experience in medical records and case management required.
- Long-term care facility experience strongly preferred.
- Knowledge of medical records regulations, HIPAA, and documentation standards.
- Strong organizational, time management, and multitasking skills.
- Excellent communication and interpersonal skills.
- Attention to detail with a strong focus on accuracy and compliance.
- Ability to work independently and collaboratively in a fast-paced healthcare environment.
