Now Hiring: LPN – Transitional Care Management
Baptist Medical Group | Collierville, TN
We’re looking for a compassionate Licensed Practical Nurse (LPN) to join our phone‑based Transitional Care Management team
!
- Call recently discharged patients to support safe transitions home
- Educate patients on medications, follow‑ups, and care plans
- Identify barriers to recovery and coordinate interventions
- Collaborate with physicians, caregivers, and healthcare teams
- Advocate for patients through clear communication and proactive planning
- Strong assessment + patient education skills
- Experience in care coordination or clinical/hospital settings (preferred)
- Excellent communication and EMR proficiency
