Quality Improvement Coordinator

SUMMARY

DESCRIPTION:

Under direction of the Executive Director, and in collaboration with the Support Plan Coordinators and Associate Coordinators, this position is responsible for:

• Development and collaboration with other staff in the preparation and development of the Quality
  Improvement Program;
• Development, cordination, and revision of Quality Improvemnt and Risk Managent Plans;
• Responding and submission of corrective action plans to DBGDS Licensing Office related to possible citations;
• Collect data and complete quarterly review of the company's progress and compliance with corrective action plans submitted to DBHDS Licening Office related to possible citations;
• Complete annual quality review and restraint & seclusion reports;
• Chair quarterly meetings with the Quality Improvement Team and
  other meetings as required;
• reporting activities,
• Preparation and participation in audits conducted by regulatory agencies regarding all quality issues and license renewals.
• Collaboration, and assit in development, revision and review company policies and procedures,
• Perform investigations and submit investigation reports to the Locacal Human Rights related to reported alleged abuse, neglect and exploitation;
• Train onboarding employees and existing employees whose trainings are scheduled to expire;
• Monitor employee's trainng due date via the company's online Learning Management System and coordinate with their supervisor to ensure employee completion of upcoming trainings that are due/past due.

• A bachelor’s degree in Human Services, Nursing or a related field (advanced degree preferred); or
• 2 years experience performing quality improvement tasks and report writing.

• Knowledge of the required elements of a person-centered plan (ISP), person-centered
  review, and support notes.
• One to two (1-2) years of supervisory or management experience pertaining to quality improvement in a behavioral health environent, preferred;
• Experience working as a coordinator/manager in a Intellectual Disability Waiver program, preferred;
• One year of experience interpreting policies and regulations, preferred;

• Knowledge of the principles and methods of process improvement, including research methodology,
  statistical analysis, and report design.

• Knowledge of Root Cause Analysis (RCA)
  methodology and reporting requirements, highly desired.

• Ability to make clear and effective public presentations about QI information, statistics
• Experience compiling data and writing reports and,
• Six month expreience providing training

• Ability to effectively supervise and coordinate the work of a workgroup engaged in quality improvement activities.

• Ability to proficiency use Windows-based PC applications with emphasis on database, word processing and
  spreadsheet software; MS Word and Excel preferred.

• Ability to interprete the concepts of DBHDS Licensing Regulations.

• Ability to plan, organize and lead complex data collection activities.
• Ability to present statistical and technical data in a clear and understandable manner utilizing
  appropriate visual aids.
• Ability to communicate effectively both orally and in writing.

• Ability to establish and maintain effective and cooperative working relationships with staff and others
  contacted in the course of the work.

• Ability to maintainaccuraterecords.

• Ability to maintain confidentiality of sensitive medical information.

• Ability to assume responsibility and exercise good judgment in making decisions within the scope of authority
  of the position.

• Ability to accurately complete tasks within established time frames.

• Ability to effectively prioritize multiple tasks and deadlines.

• Valid Virginia Driver License, transportation, and automobile liability insurance.
• Certification as a Human Rights Investigator provided once employed by the company.
• Certification of other trainings, as required.
