Licensed Practical Nurse – Home Care
As the Licensed Practical Nurse, you will provide direct nursing care as prescribed by the physician to acutely or chronically ill patients in their homes and record clinical and progress notes. You will coordinate other patient care activities to deliver the highest quality of patient care in the geographic service area assigned to you. You will work under the direct supervision of the Registered Nurse and/or designated supervisor.
PrimaryResponsibilities
- Provides high quality clinical services within the scope of practice, as defined by the state laws governing the practice of nursing, in accordance with the plan of care and in coordination with other members of the health care team, following all applicable infection control procedures.
- Documents patient visits thoroughly and completely per regulatory and payer requirements in the electronic medical record utilizing the agency's electronic visit verification program (as applicable).
- Actively participates in the agency's care coordination process including timely reporting of patients' needs and changes in condition, attending patient case conference, communicating effectively to the patient, family, physician, other clinicians, agency management staff, case coordinator, community case managers (as applicable).
- Be able to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.
- Current and unrestricted Licensed Practical Nurse licensure in the state of Oregon.
- Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
- Current CPR certification or ability to complete within 90 days of hire.
- Demonstrated ability to work flexible hours.
- Demonstrated ability to manage multiple tasks simultaneously.
- Demonstrated ability to work independently.
- Proven good communication, writing, and organizational skills.
Pay is based on several factors including local labor markets, education, work experience, certifications, etc. The hourly pay for this role ranges from $24.10 to $36.15 per hour for full-time employment. In addition to your salary, we offer a comprehensive benefits package, incentive and recognition programs, equity stock purchase, and 401(k) contribution (subject to eligibility). We comply with all minimum wage laws.
