In-Home Health - Nurse Practitioner or Physician Assistant; -Greeley

Overview

Signify Health, part of CVS Health®, is hiring a full‑time Clinician (Nurse Practitioner or Physician Assistant) to provide in‑home health evaluations and related diagnostic services.

• Conduct about 45‑minute home visits, reviewing medication and medical history, performing physical assessment, and ordering optional diagnostic tests (e.g., diabetic eye exams, spirometry) as requested by the member’s health plan.
• Perform diagnostic and preventive services, including spirometry, DEE, and other indicated evaluations.
• Conduct virtual visits as necessary.
• Travel locally and within licensed states to visit members nationwide, with primary coverage area of Greeley and surrounding area.
• Join a national network of clinicians dedicated to improving health outcomes.
• Not required to prescribe medication, order laboratory tests, or alter a member’s current treatment regimen.

Primary workplace is the private residence of health plan members. Environments may contain household pets, secondhand smoke, and potential airborne allergens; candidates must be able to perform duties safely in such conditions.

• Active, unrestricted Nurse Practitioner or Physician Assistant license(s) in coverage area(s).
• Board certification (required).
• Multi‑state license(s) preferred.
• Ability to travel locally and within licensed states, including primary coverage area of Greeley and surrounding area.
